Building Healthier Communities


A health care system that doesn’t work for patients, seniors and families is a health care system that just isn’t working at all. Your PC Government is taking action to end hallway health care and ensure that all Ontarians can receive the right care in the right place.

Earlier this week, Christine Elliott, Deputy Premier, Minister of Health and MPP for Newmarket-Aurora, announced the PC Government’s plan to improve the home and community care system by enabling integrated and innovative models through the introduction of the Connecting People to Home and Community Care Act

The legislation, which is part of the PC Government’s comprehensive plan to put an end to hallway health care, would modernize the delivery of home and community care and help ensure patients receive the care they need as quickly and conveniently as possible.

“We are breaking down long-standing barriers that have separated home care from primary care and, in doing so, allowing for the seamless coordination of services for patients, while maintaining and strengthening oversight and accountability measures,” said Elliott.

If passed, the new legislation would:

  • Make it easier for people to access home and community care in hospital, primary care or community settings.
  • Help people connect with their care providers through secure video conferencing and remote monitoring devices.
  • Provide more choice for people with high care needs to get care in new community settings.
  • Keep people healthier at home by empowering care teams to work together in making decisions about and integrating care.
