Ontario PC Government Connecting 300,000 More People to Primary Care

Your Ontario PC Government is delivering on its plan to connect more people to the care they need, closer to home. Last month, the first call for proposals was launched to create and expand up to 80 primary care teams — helping connect 300,000 more people to a family doctor or primary care team this year.

This is a key milestone in the government’s action plan to ensure that every person in Ontario is connected to primary care by 2029, no matter where they live and builds on the 78 new and expanded teams created over the last year which connecting 300,000 more people to primary care.

The province is investing $213 million to support this first call for proposals. This funding is part of a historic $1.8 billion investment to add 305 new and expanded primary care teams across Ontario, connecting two million more people to publicly funded primary care over the next four years.

The first call is targeted to communities with the greatest need, focusing on postal code areas with an average of 8,000 people currently not connected to primary care. This targeted approach will ensure those who need care most are prioritized and supported.

Primary care teams — including family health teams, community health centres, nurse practitioner-led clinics, and Indigenous primary health care organizations — are invited to apply by May 2, 2025. Teams must demonstrate how they will attach the maximum number of people within their identified postal codes. Selected applicants will be announced this summer, with a second call for proposals launching in September.

To support these efforts, the government is also:

  • Investing $22 million to help existing primary care teams manage operational costs;
  • Investing $37 million to support Ontario Health Teams and their Primary Care Networks in delivering automatic patient attachment by postal code.
